We understand. If you’re new to the concept of Direct Primary Care and still need to talk it through, we’ve got you! Click the link below to schedule your 15-minute complimentary phone call. We’ll answer your lingering questions about DPC, and if you like what you hear, we’ll add you to our waitlist to be scheduled for your intake appointment when we officially open. If you’re a former patient of Dr. Holt’s, or need scholarship-based membership options, please let us know so we can talk discounts.
Initial Consultation
Once we officially open, we’ll send you some forms to fill out and collect your non-refundable enrollment fee (unless you're in that lucky first bunch of patients we are waiving that for). We will also reach out to schedule you for an intake visit, or an initial consultation. This first visit is a review of your intake forms, your medications (and whether we can save you money on your medications by getting them at wholesale prices), touring our clinic and reviewing how it all works. This can be done via telemedicine, but we encourage you to visit our beautiful space. At the time of your intake appointment, if the shoe fits, we’ll start your monthly membership. We’ll schedule your first exam at a time convenient for you, whether that’s within the month, in 3 months, or even later that day.
First Exam.
This could be the day of the initial consult, or some other time within the month. It includes a comprehensive exam, and possibly labs and medication dispensed if you need it. Don't forget, we don't accept insurance so you don't need to worry about timing your preventive visit just right for insurance to cover it. Your first exam, and every other visit, is included in your monthly membership fee.
